Upon completion of the elibility screening process using HelpWorks, the input collected from your client can automatically populate the forms required by service providers. Once the forms are created, the care coordinator can print them out and have them signed by the client so they can be delivered by the client to the provider that will provide a service. The forms can also be stored within the HelpWorks database and retrieved by any agency, with appropriate permissions. They can also be emailed. They can be updated as information from a client changes.
eForms will save time and improve the accuracy and efficiency throughout the process of service delivery. What follows are two forms used throughout the State of New Hampshire. We're sure you'll recognize them. They are in the system and ready for use. Other standard forms will be added or custom forms can be developed by you or NH ParTech to meet your specific needs.
